Key Principles of Melasma Treatment at Dr. E-Laser Dermatology
Melasma varies depending on skin depth and underlying causes.
Customized Laser Settings for Every Type of Melasma
Melasma varies in pattern, depth, and distribution.
At Dr. E-Laser Dermatology, each treatment begins with a precise skin diagnosis,
allowing us to adjust the laser energy level, wavelength,
and number of passes for every session.
This customized approach ensures safer and more effective melasma treatment
tailored to each patient’s skin condition.
Sufficient Treatment Time for Optimal Results
Successful melasma treatment is not achieved by technique alone.
Accurate treatment settings must be combined with careful application and sufficient treatment time.
At Dr. E-Laser Dermatology, the focus is not on speed, but on precise and thorough laser treatment, delivering results through controlled and carefully performed procedures.

Why Melasma Treatment Is Challenging
Melasma is not a simple pigmentation condition. Its exact cause has not been clearly identified
and is believed to involve multiple complex factors, including UV exposure,
damage to the epidermal barrier, hormonal influences, and immune responses.
Because of this complexity, melasma treatment requires a very different approach from treating freckles or dark spots.
In the treatment of melasma and stubborn pigmentation, the most important factor is not the device itself, but the precise treatment parameters.
Understanding Melasma Treatment Parameters
Effective melasma treatment begins with accurately identifying the depth,
distribution, and characteristics of pigmentation.
Unlike simple dark spots, melasma often spreads diffusely across the surrounding skin.
If the surrounding pigmentation is not carefully treated, the skin may appear uneven or
the pigmentation may worsen. Melasma treatment is therefore not just about removing pigment,
but about carefully controlling the skin’s response through precise laser treatment parameters.
For this reason, successful melasma treatment requires experienced physicians
with extensive clinical expertise in laser dermatology.
Types of Melasma
Treatment Methods Vary Depending on Pigment Depth
Post-Inflammatory / Treatment-Induced Melasma
This type of melasma may occur or worsen After
laser treatments or external irritation, resulting in conditions such as
post-inflammatory
hyperpigmentation, hypopigmentation, or rebound pigmentation.
Because both the epidermis and dermis may be involved due to irritation, heat, or
inflammation, this type tends to be highly reactive and prone to recurrence, requiring
particularly careful treatment.
Mixed-Type Melasma
Mixed melasma is the most common type, where pigmentation exists
in both the epidermis and dermis. It is especially common
among Asian patients. Because pigment depth varies, a comprehensive and carefully
customized treatment approach is required.
C-Pattern (Nike-Pattern) Melasma
This type appears along a curved C-shaped pattern,
starting from the cheekbone area and extending toward
the temples and outer eye line. Since pigmentation often
involves both epidermal and dermal layers, treatment responses may vary depending on the
specific area.
Precise Analysis of Melasma Depth
MarkVu Skin Analysis
With MarkVu skin analysis, we can closely examine not only visible
pigmentation, but also hidden pigment, pores, redness, and skin texture.
Using multiple light sources such as UV, polarized light, and RGB
imaging,
MarkVu analyzes the skin in detail from the epidermis to the dermal
layer,
allowing us to accurately identify the distribution and depth of
pigmentation.

A Melasma Treatment Protocol Based on Depth, Cause, and Skin Condition
From precise diagnosis to post-treatment maintenance,
each step of the process is carefully structured
to reduce recurrence and achieve a clearer, more even skin tone.
Diagnosis & Skin Conditioning
Using the MarkVu skin analysis system, we carefully assess the depth and distribution of pigmentation, skin tone, and vascular condition.
Based on the diagnostic results, we first stabilize the skin barrier and overall skin condition,
preparing the skin for safe and gentle laser treatment with minimal irritation.
Customized Laser Program for Melasma
Laser treatments are selected based on the type of melasma (epidermal, dermal, or mixed) and the patient’s skin thickness, ensuring minimal skin damage.
The treatment gradually breaks down pigmentation and evens out the overall skin tone through carefully controlled laser procedures.
Maintenance Care to Prevent Recurrence
After treatment, we provide maintenance programs designed to reduce the risk of melasma recurrence.
Through regular skin analysis and follow-up care, we monitor the skin condition and help maintain clearer and more stable skin tone over time.

Melasma Treatment Case (Patient in Their 40s)
This patient visited our clinic after receiving melasma treatment at another clinic, where
the pigmentation had become darker after treatment.
Severe pigmentation was visible under the eyes and across the cheekbones.
After completing 10 sessions of a customized melasma treatment program, the patient showed
significant improvement.
The patient now continues maintenance treatment every 4–6 weeks to maintain stable results.
Ota-like Nevus, Melasma & Pigmentation Case (Patient in Their 40s)
This patient developed round pigmented lesions on both cheekbones beginning in their 20s.
Diagnosis revealed Acquired Bilateral Ota-like Nevus (ABNOM) combined with melasma and
additional pigmentation,
forming a mixed pigmentation condition.
Treatment was performed using two types of laser therapy, combining Q-switched Ruby Laser
and Q-switched Nd:YAG Laser.
Melasma & Pigmentation Case (Patient in Their 50s)
After pregnancy and childbirth, pigmentation began appearing under the eyes and gradually
darkened over time.
The patient initially used over-the-counter melasma creams without improvement
and even developed areas of hypopigmentation.
Nike-pattern melasma appeared under both eyes.
After approximately 20 sessions of melasma treatment, the patient achieved significant
improvement.
Severe Melasma Case After Multiple Treatments (Patient in Their 50s)
Melasma developed after pregnancy and initially improved with laser treatment.
However, the pigmentation later recurred and became darker.
A symmetrical C-pattern (Nike-pattern) melasma appeared from the lower eyelid to the
temples.
Following a customized melasma treatment program, the pigmentation cleared significantly and
overall skin tone improved.
Severe Melasma Case After Multiple Treatments (Patient in Their 50s)
This patient visited after receiving more than 30 treatments at another clinic, yet the
condition had worsened.
Pigmentation had spread widely across the under-eye area, cheeks, mouth area, chin, and
forehead.
Through Dr. E-Laser Dermatology’s melasma treatment protocol, pigmentation improved significantly,
with additional improvement in skin texture and pore appearance.
Long-Term Melasma Case (Patient in Their 40s)
The patient had received melasma treatment for three years at another clinic without
improvement.
Even melasma injections and prescribed supplements showed no effect.
Severe pigmentation was present under the eyes and across the cheekbones.
After 20 sessions of customized laser melasma treatment based on pigmentation depth and skin
tone,
the patient achieved significant improvement and continues periodic maintenance treatment.
